| Licensed Home Day Care In Claymont,Delaware Certified in CPR, First Aid, and Administering Medication Single Family Home with large fenced in yard. No Pets. No Smoking. Hours of operation are between 7:15 AM-- 5:15 PM Monday- Friday. Cost: Full time is $135.00 per week Includes: Pre-school program from Sept.- May also includes Breakfast, Lunch, and a snack for toddlers. Schedule of Payment: Payment is to be made on the first day of the month. Holidays: The following is a list of holidays that I am to be paid and my family daycare will be closed. If your regularly scheduled day falls on a closed holiday you are responsible for payment of the holiday. You may not switch your day in order to avoid paying for the holiday. Also, remember that it is your responsibility to find alternate care for your chid when I am closed for the holidays. You will be given a list each year listing the exact dates of the holidays that I am closed. New Years Day Memorial Day Independence Day Labor Day Thanksgiving Day Friday after Thanksgiving Christmas Eve Christmas Day Provider Vacation / Personal Time: My family daycare home will be closed for two to three weeks each year for my personal vacation/ personal time with my family. There will be no charge to you for these weeks. You will be given the dates for my vacation well in advance so that you can possibly plan your vacation to coincide with mine. Also remember that it is your responsibility to find alternate care for your child when I am closed. Provider Sick: You will not be required to pay for my sick time. I will try to notify you as soon as possible if I am unable to care for your child due to myself or my son's illness. Also remember that it is your responsibility to find alternate care for your child when I am closed. Child Vacation/ Sick Time: If you need to take any additional days/ weeks of during the year you will be charged for these days. Please keep your child home if he/she is too sick to be in my home with other children pick him/her up promptly when called to do so. If your child is out (sick) for 3 days or more I require a physican's statement before they can return to my family daycare home. We have read the information in this package and agree to the terms and conditions. By signing this contract we agree to abide by these terms and conditions. The provider may amend the policies by giving a copy of the new or changed policies at least one month before they go into affect. ______________________ ______________________ I am a Mother myself and I know how hard it is to find good care for your child. I wanted to be able to stay home with my son and at the same time provide other parents with a safe and loving enviroment for there children. I will make your son/daughter feel safe and loved as if they were in there own home.
